At Nevada Orthopedic & Spine Center, our mission is to make a difference in your health care experience. We provide an array of patient information to better serve you but if you have any questions or need further information, please do not hesitate to contact us.
Nevada Orthopedic & Spine Center accepts patients by referral or without referral, according to your healthcare insurance plan. Click here to view a list of the insurance plans that we accept.
Appointment times are based on medical need and patient convenience. All minors under 18 years of age must be accompanied by a parent or legal guardian in order to be treated by our physicians. All new patients are required to arrive 30 minutes prior to their appointment time.
Please Bring the Following To Your Visit
- Photo ID
- Insurance Cards
- All Diagnostic Tests - i.e., MRI, X-rays, CT scan
- A list of all current medications
- EMG Report for Carpal Tunnel Syndrome patients
- Medical Records from other Treating Physicians
Rescheduling/Cancelling Your Appointment
We understand that your schedule may change but we ask that you contact us immediately to reschedule your appointment. We appreciate as much notice as possible to reschedule your appointment so provide more appointment opportunities. There is a $25.00 charge for appointments cancelled less than 24 hours prior to the scheduled appointment time. This fee is also applied to your account if you fail to show for a scheduled appointment.
If you require forms to be completed by your physician at your appointment, please be aware of the associated fees and time lines. Payment for these forms is required at the time of your request.
- Disability Forms: $15.00 per form
- FMLA Forms: $20.00 per form

Nevada Orthopedic & Spine Center understands that medical information is private and we are committed to protecting your personal information. Nevada Orthopedic & Spine Center will not disclose your information to anyone without your signed written consent.

In the case of a medical emergency, please call 911. You have the right to request an orthopedic referral from the emergency room to a Nevada Orthopedic & Spine Center physician.
